THINK before you speak

You've probably heard this phrase before: "Think before you speak". Yes, of course you should pause before you say anything rash, inconsiderate, or inappropriate. That's common sense, right? And yet this seems to be surprisingly hard to commit to even though many of the difficulties in life can be solved (or avoided altogether) by using this bit of wisdom being taught at the earliest levels of education. We seem to "grow up" and forget how to behave.
